The Community Case Management Shelter Intake is a centralized intake process and waiting list for Snohomish County. This innovative program began taking calls from homeless individuals and families on September 4, 2007.
How it works Anyone in Snohomish County looking for emergency shelter can call 425-212-2900 or 2-1-1. You can also reach us through Email at: CCM@voaww.org. A Housing Intake Specialist will complete an intake questionnaire and place the individual or family on a centralized waiting list that all community shelter providers will access when placing households into their shelter programs.
Housing Continuum Model
Quality Assurance To insure the overall accuracy of the data and intake process of Community Case Management, the program is overseen by the Program Manager. They work with Emergency Housing Providers to insure placements of clients as well as develop policies and procedures to strengthen the overall effectiveness of the program. Additional responsibilities include training the Housing Provider Staff on software systems, contacting clients when requested by providers to expedite housing placement, and providing accurate data on the client population when requested from partner agencies. In addition, the Program Manager participates in community planning and networking as it pertains to housing advocacy and program-related support.
