Eligibility Criteria
Discover if you're eligible for our services by checking the criteria listed below.
Supportive Housing

Must be 18 or older
-
Currently receiving Medicaid (WA AppleHealth) insurance
-
Have a series of complex needs
-
Be experiencing homelessness or having multiple stays in certain inpatient settings.

AND
-

Have a mental health diagnosis, substance use diagnosis, or a physical impairment.

Supported Employment

Must be 16 or older
-
Currently receiving Medicaid (WA AppleHealth) insurance

Have a series of complex needs

Be experiencing social, physical, or economic events that cause you to struggle with finding or maintaining employment

Have a mental health diagnosis, substance use diagnosis, or a physical impairment.
